bermacam macam isu berlaku, bermula dari zaman penulis berada di malaysia..sampailah penulis berada di bumi anbiya’ ini, penulis masih tidak berpuas hati dengan sikap beberapa orang sahabat yang dianggap seperjuangan, tapi rupanya tidak..masing2 saling memusuhi apabila sesama sahabat tidaklah sefikrah, se amal, dan seHARAKI.

KHUTBAH AL-WIDA’
Pesanan Dan Peringatan Penting Buat Umat Islam dan Pemimpin Mereka

Di sebalik amaran dan ancaman penyakit selesema H1N1, musim haji kali ini tetap berjaya menghimpunkan jutaan umat Islam bagi melakukan suatu pengorbanan yang besar di sisi Allah SWT. Namun demikian, terdapat satu amaran lain ke atas umat Islam dalam menunaikan ibadah haji tahun ini. Ia merupakan satu ancaman yang datangnya dari ‘penjaga’ haramain (pemerintah Saudi) itu sendiri, di mana beliau mengancam umat Islam agar tidak menjadikan pertemuan besar di tanah suci itu untuk membincangkan perkara-perkara politik. Sungguh hina apa yang dilakukan oleh si pengkhianat umat ini! Beliau melarang umat Islam dari membincangkan urusan politik walhal politik itu adalah sebahagian dari Islam. Siapa tak kenal Raja Abdullah yang sesungguhnya merupakan penjaga kuffar Amerika dan langsung tak layak menjadi penjaga rumah Allah yang suci.
Tidak ada seorang manusia pun yang berhak menghalang manusia lain dari membincangkan perihal Islam, apatah lagi di tanah suci, baik dari segi perbincangan politik, ekonomi, sosial, ibadah dan sebagainya. Sesungguhnya di tanah suci inilah Rasulullah SAW dahulu memulakan perjuangan politik baginda di mana para sahabat diseksa dan dibunuh dalam usaha mereka membebaskan rumah Allah ini dari segala taghut dan kekufuran untuk digantikan dengan hukum dan sistem Islam. Setelah 13 tahun perjuangan berlangsung di Mekah, Rasulullah dan kaum Muslimin kemudian berhijrah ke Madinah dan peristiwa hijrah yang penuh bersejarah itu telah menjadi titik perubahan kepada landskap politik dunia di mana Daulah Islam yang pertama telah ditegakkan dan agama Allah buat pertama kali telah diterapkan dalam bentuk bernegara.

Mr. Ahmad, male, age 52 years old, Pakistani, has history of throat infection, malaise, skin rash, swollen and tender joints during childhood. He was accompanied by his 14-yrs old daughter (as the interpreter) to the GP clinic.

Case 6 S3 Thrombosis

Summary of the Case:

 Clive, 20 years old male, weight is 133kg and height 1.75m (BMI= 43.43). Admitted to hospital by ambulance after collapsed at his home. According  to the patient, he had constipation during defecation, and suddenly felt a severe crushing pain on his chest, breathless and then lost his conscious. History from patient, a smoker, take large amount of dinner and frequently ate fast food. Impression at A & E: heart attack. Investigation already done consist of ECG, Arterial Blood Gases (ABG), FBC, Cardiac Enzyme test, PT/INR.

Case 5 S3 Anaemia 9 November 2009

Summary of the Case:

Sarah, 28 years old woman, is a keen blood donor. No history of anaemia and any blood borne disease. Her blood group is AB positive. She donated blood since ten years ago, which she had to donate three times for each year. One day, the blood bank called her and told her that there were some abnormal monocyte in her blood. Suspecting that she had leukaemia, she went to a GP to consult her condition. An investigation done which revealed that her blood level of haemoglobin is very low, 8.0 g/dL (normal 13.0-17.0). MCV is 72fL (normal 83-101). Serum ferritin also low.  Sarah is having menorrhagia lately. She has been diagnosed to have infectious monocytosis (glandular fever). Now under treatment of hormone preparation to reduce heavy periods  and ferrous sulphate-tab to restore her iron blood levels.

A Gift for A Doctor

On 11th September 2009, it was my last day posting in Department of Orthopaedic, Tengku Ampuan Rahimah Hospital (HTAR), Klang. The deep feeling of sorrow can be felt in my heart for leaving the hospital which I was taught a lot to be a professional and competent doctor. As my last day, I’ve requested for on call shift from my department, and they granted me. As usual for weekends, emergency department will always be filled with fresh patient, fresh blood and also fresh doctor, whose recently graduated from medical school around the world and have to tag themselves to another doctor for clinical exposure. But on that day, there was no fresh doctor, only two housemen and me as an experienced medical student..chewah..

Suddenly at 11 pm, lot of cases arrived. Being in Emergency , A & E Blue Zone, which belonged to Orthopaedic Department, the cases that arrived here normally and must be related to musculoskeletal injury. So, lot of x-rays, limbs and so on have to be examined. During that night, I cannot deny that we, the doctors, were extremely exhausted. Being worked for nearly 24 hours, that crazy, patients screaming, nurses babbling around and the specialist or MO come to observe us..only person that really have “ jiwa seorang doktor” can face this. Before 9pm, there were some nursing student that helped us to reduce our workload, so for me..it was easier for me to manage the ward, I just have to ask for their help “ adik, amik SPO2, adik tolong bacakan SPO2, adik amik set Irrigation, adik itu adik ini “..later after that, all these work have to be done by ourselves. We do the investigation, we plan for management, we diagnose the patient, and we also have to prepare all those thing, I mean all medical instrument. Imagine, 2 housemen, a medical student, 4 staff nurses to manage a ward that was so hectic just like world war 2 field hospital. Only God knows how terrible it was.
